Form C - Missing or Damaged Property Notification
The "Form C - Missing or Damaged Property Notification Form", or "Missing Report", is utilized by Property Management to record any and all missing and/or damaged departmental assets. Form C must be filled and signed by the appropriate personnel, i.e. the department head or department designee, in order to facilitate proper reporting of missing and damaged assets. Note that this report does NOT require an incident report from the University Police Department.
